2015, Seminars in Nuclear MedicineCitation Excerpt :Among the mechanisms proposed was that, in the presence of a LBBB, an increasing HR results in a smaller proportion of diastolic coronary flow time with septal hypoperfusion. Burns et al44 hypothesized a mechanism of early septal contraction encountering only right ventricular outflow resistance resulting in lower septal contractile tension and thus less blood flow. Proposing that this scenario would not occur with vasodilator “stress,” this group saw that in the setting of LBBB, compared with exercise, dipyridamole stress improved image specificity from 20%-90% (P < 0.05).

Several of the effects mentioned that may lead to false positives are caused by the increase in cardiac inotropy and chronotropy and so vasodilator stress is preferred to exercise or pharmacological in patients with LBBB [25–27]. O'Keefe et al. reported a specificity of diagnosing septal CAD of 82% using adenosine compared to 42% with exercise stress [26].
